What is a Short Scar Facelift?

A short scar facelift or mini facelift is a type of facelift that addresses jowling and lax skin around the jawline. When performed as a deep plane mini facelift, the ligaments under the SMAS or muscle are released to get a better lift. This will also tighten the neck in patients with mild to moderate neck laxity. Those patients with significant neck laxity or banding may require a neck lift as well. The mini facelift or short scar facelift is performed as an outpatient and recovery is about 2 weeks.

Is General Anesthesia Required for Rhinoplasty

Rhinoplasty is typically performed under general anesthesia. There are a few reasons for this. The nose is a quite sensitive area and despite utilizing lots of local anesthesia during rhinoplasty, injection of this initially into the nose is uncomfortable if you are awake or even sedated. Not to mention, it is impossible to completely anesthetize the entire nose with local anesthesia. In rhinoplasty, frequently, controlled osteotomies are performed to narrow the nasal bones and most people do not want to be awake when someone is using a mallet and osteotome (chisel like instrument) on the bones of their face.

Lastly, during rhinoplasty, there is bleeding from the nose which will drip into the throat. If you are sedated, that blood could enter the airway and you could aspirate leading to airway complications. With general anesthesia, there is a breathing tube in the airway and guaze in the back of the throat to prevent blood from entering the airway or the stomach (which can cause nausea).

For these reasons, most rhinoplasty is done under general anesthesia.

Neck Lift for Men

The most common complaint from aging men is their saggy neck, turkey neck, or neck fat. Depending on the patient’s age, skin elasticity, and amount of fat present, the appropriate treatment may vary.

Men in their 30’s and early 40’s may benefit Kybella injection to this region to decrease neck fat. This requires at least 3 injection treatments 2 months apart. Downtime and discomfort are very minimal and you will simply have some swelling for 3-5 days after the first round of injections. The other option for men this age you have good skin elasticity is submental liposuction. The advantage to neck liposuction versus Kybella is that you can define the jawline more with submental liposuction and cannot with Kybella. Recovery from liposuction is 5-7 days.

Men in their late 40’s and above may require a neck lift. In this situation, liposuction or direct fat excision would be performed followed by suspension of the platysma muscle behind the ear to tighten the neck. This would require surgery as an outpatient and recovery is about 10-14 days.

Eyelid surgery and Rhinoplasty

A common question from my Houston patients is whether they can have eyelid surgery with rhinoplasty. Eyelid surgery can be performed with rhinoplasty. Whether it is upper eyelid surgery or lower eyelid surgery for lower eyelid bags. Either of these or both can be performed at the same time as rhinoplasty.

There may be more swelling as the eyelids will typically bruise slightly and swell with rhinoplasty alone. The combined eyelid surgery and rhinoplasty will likely cause a little more swelling and bruising.

Neck lift or Facelift or Both?

Many of my Houston facelift, mini facelift, and neck lift patients are confused by the terminology and procedures regarding facial rejuvenation surgery. Those patients who only want a tighter neck may benefit from submental liposuction or neck liposuction. If there is significant neck skin laxity, a neck lift may be warranted.

If you simply have jowling and want a tighter jawline but have minimal neck issues, a mini facelift or short scar facelift may be appropriate for you. There is less scarring, shorter recovery time, and can be performed on an outpatient basis.
