Should I get rhinoplasty with septoplasty?

Many of my Houston patients seeking septoplasty for a deviated septum are interested in making changes to the appearance of their nose at the same time. Rhinoplasty or nose job surgery can be performed at the same time as septoplasty and turbinate reduction. In fact, almost every rhinoplasty typically requires some sort of septoplasty as the cartilage from the septum is utilized for grafting purposes during the rhinoplasty. This cartilage may be used to straighten a crooked nose, support the nasal tip, refine the nasal tip, or build up the nasal bridge. If you have the septoplasty separate from the rhinoplasty it may complicate future rhinoplasty if you are considering making changes to the appearance of your nose. There may be insufficient cartilage available in the septum requiring ear or rib cartilage for your rhinoplasty.

Scars after nostril narrowing

Patients with wide nostrils such as ethnic rhinoplasty patients frequently ask about the scarring around their nostrils. The scar is placed in the nasal sill and then around the junction of the nostril with the face in a shadowed region. This scar is easily hidden with makeup at 2-3 weeks but will take 3-6 months to be imperceptible without makeup. This technique is frequently used in ethnic rhinoplasty or in patients with flared nostrils.

Do I have to have both upper and lower eyelid surgery?

Patients presenting with concerns for their upper or lower eyelids may wonder if they HAVE to address both if they are mainly concerned with one. The answer is not necessarily.

Upper eyelid surgery will create a less heavy upper eyelid and allow a more open eyed appearance. This give a more alert and awake appearance and allows patients to more easily place eyeliner, eye shadow, and mascara.

Lower eyelid surgery will address the heavy bags, dark circles, and loose skin of the lower eyelid. This will give a more rested, refreshed, and youthful appearance.

Together, upper eyelid surgery and lower eyelid bags surgery will complement one another but patients have to choose which bothers them most and have a frank discussion with their surgeon regarding the best treatment regimen for their eyes.

Will a Mini Facelift correct my neck?

Many of my Houston facelift patients come in wondering if a mini facelift with correct the concern they have with their neck. A mini facelift will help correct some aspects of the neck depending on two factors.

Factor number one is the severity of the neck. Patients in their 60’s or 70’s with extensive skin laxity of the neck or patients with a heavy waddle or large neck may benefit slightly in the neck with a mini facelift. These patients will likely require direct fat excision, possible platysmaplasty, and possible neck lift. This requires elevation of the skin of the neck and suspension of the platysma muscle to the mastoid bone behind the ear.

Factor number two is the extent of the mini facelift. The mini facelift’s purpose is to improve the jowls, jawline, and perhaps the mid face. The SMAS is a layer of fascia that lies over the face and is the extension of the platysma muscle in the neck. If a deep plane dissection is conducted with the mini facelift and the sub SMAS ligaments are released, this will help suspend the neck better and tighten it at the same time.

For patients with mild neck skin laxity and small amounts of neck fat, a deep plane mini facelift will help correct concerns the patient may have with the neck.

Does a Facelift Include the Eyes?

Facelift surgery or a mini facelift typically addresses the jowls, jawline, and some of the neck. The deep plane aspect of a facelift will also lift portions of the mid face. The neck can be addressed with a neck lift or face and neck lift.

Eyelid surgery addresses excess upper or lower eyelid skin and also can treat the lower eyelid bags. Upper blepharoplasty removes excess skin from the upper eyelid and provide a more rested refreshed appearance to the eyes. Lower eyelid bags surgery removes excess fat and creates a softer more rejuvenated look while tightening the skin as well.

Eyelid surgery is a separate procedure from a facelift. Eyelid surgery and facelift surgery are frequently performed at the same time.
