Eyelid surgery versus filler under the eyes for lower eyelid bags

Many of my Houston eyelid surgery patients are concerned about their lower eyelid bags and want to know the best treatment for this.

As we mature, we lose collagen. There is a wall of collagen that holds the fat back that surrounds our globe or eyeball. As that collagen wall weakens, the fat begins to push out and form the “bag” under the eye. The shadow formed from the bag leads to the dark circles under the eyes.

If the bags under the eyes are severe, surgery may be your only option to address these bags. These bags can be simply removed along with removal of some skin. Sometimes, the fat can be mobilized and placed in the shadowed area beneath the bags. This will lead to a smoother transition between the eyelid and the cheek junction.

If the fat pads are quite small and the lower eyelid is already a little recessed, than a lower blepharoplasty or eyelid surgery will lead to an even more hollowed appearance and an unpleasing aesthetic result. In these cases, added volume is necessary and filler should be used to fill the tear trough and blunt the appearance of the fat or under eye bags.
