Filler or surgery for lower eyelid bags

A common complaint from my Houston patients are lower eyelid bags and/or dark circles under the eyes. The lower eyelid bags are due to fat surrounding the eye slowly bulging over time below the eye in the lower eyelid. The majority of light that hits our face comes from above and this fat will cast a shadow creating the dark circles under the eyes. The fat bulging also creates a depression in the area we call the tear trough of the lower eyelid.

This tear trough and shadowed area can be corrected with filler injection or surgery. With filler injection, a hyaluronic acid filler such as Restylane or Juvederm can be injected to correct this issue. This is done in the office and tolerated well with little to no downtime. The other option is lower eyelid surgery for lower eyelid bags. Here, some of the excess fat which causes the “bags” under the eyes is removed and can be repositioned into the tear trough. This is a surgical procedure and requires anesthesia. Recovery after lower eyelid surgery is about 8-10 days.

Will a facelift improve my marionette lines?

Many of my Houston facelift patients will come in asking what a facelift or neck lift will do for them. More specifically, which wrinkles and lines will it improve. The mini facelift or short scar facelift and the full facelift with neck lift will significantly improve the marionette lines or lines that extend down from the corner of the face. These lines will improve about 80% with a facelift. The smile lines or melolabial lines will improve about 20-30% with a facelift depending on the extent of dissection and the type of facelift (deep plane facelift will improve these). The fine lines in the cheek will improve as the deep muscle and fascia is lifted and suspended. The excess skin will then be removed leaving a smoother appearance to the skin.

Revision rhinoplasty after septoplasty

Many of my Houston revision rhinoplasty patients had their initial rhinoplasty by a surgeon with ENT training. If this is the case, their original surgery likely entailed a septoplasty for a deviated septum. Septoplasty requires removal of cartilage and bone from the septum. This cartilage is commonly needed for grafting purposes during rhinoplasty and revision rhinoplasty. If there is insufficient septal cartilage remaining from the previous septoplasty, then the rhinoplasty surgeon may need to use ear cartilage or rib cartilage. Frequently, there is residual cartilage remaining after a septoplasty and this can be salvaged for the rhinoplasty or revision rhinoplasty.

Upper eyelid surgery and permanent eyeliner tattoo

When discussing eyelid surgery with my Houston area patients, it is important to review the location of the incisions for upper eyelid surgery and lower eyelid surgery. The incision should not disorient the tattoo or break it up. The scar for upper eyelid surgery should lie in the upper eyelid crease and then in one of the crow’s feet creases. The lower eyelid incision and scar should end up 2mm below the lash line and therefore about 1mm from the lower eyelid liner tattoo.

Digital imaging with rhinoplasty

Many of my Houston rhinoplasty patients who come in for consultation are interested in seeing digital imaging to get an idea of what their nose may look like after rhinoplasty. Digital imaging of the nose for rhinoplasty can help a patient also understand what the surgeon is capable of and what THEY are thinking for your nose.

It is also important to note that making changes to the frontal view is difficult with digital imaging. A photo is only in two dimensions while the nose is a three dimensional object. Making changes to lights and shadows on the nose for rhinoplasty can be difficult to mimic an exact surgical result.
