Will rhinoplasty change my eyes?

Many of my Houston rhinoplasty patients will ask if nose surgery will change other aspects of their face. Rhinoplasty will not necessarily change the shape of the eyes. However, it may lead others to focus more on your eyes. A crooked nose, bulbous tip, or nasal hump can draw attention away from the features of the face we want people to focus on such as the eyes, lips, and hair. Creating a softer, more subtle, and natural appearing nose will allow people to not be distracted by the nose and therefore more focused on the other features of the face. Narrowing a wide nose will also give the illusion that the eyes may be further spaced apart.

Can revision rhinoplasty be closed?

Many of my Houston revision rhinoplasty patients who present for consultation will have had a closed rhinoplasty during their previous procedure(s). Some revision rhinoplasty patients will ask if another closed procedure could be performed for their revision rhinoplasty.

Open rhinoplasty requires a mid columella incision that heals quite well. Open rhinoplasty allows for better visualization of the anatomy and more precise placement of sutures and cartilage grafts. This is critical with revision rhinoplasty as the surgeon and the patient wants to assure the patient obtains the best possible result. A closed revision rhinoplasty should only be considered if simple osteotomies need to be performed or the dorsum simply needs to be rasped.

Lower eyelid surgery and rhinoplasty

As some of my Houston rhinoplasty patients may be in their 30’s or 40’s, they are interested in surgery for bags under the eyes. These patients question whether lower eyelid surgery can be performed at the same time as rhinoplasty. Rhinoplasty and lower blepharoplasty can be safely performed at the same. I usually do the lower eyelids first as this is a “cleaner” or more sterile surgery than the nose job surgery. I explain to patients that most rhinoplasties get some bruising around the eyes as do lower eyelid surgery so from a recovery perspective, this is the time to do both. However, the two combined usually causes more swelling of the eyelids. Nevertheless, recovery from combination rhinoplasty and lower eyelid bag surgery is usually 10 days until you are social.

Can a facelift and rhinoplasty be performed at the same time?

Many of my Houston facelift and rhinoplasty patients are interested in having multiple procedures in order to have one recovery and one general anesthetic. Some of my Houston facelift patients are interested in having rhinoplasty during their facelift or neck lift procedure. Although these are both complex and combined can last several hours, it is possible to perform these procedures at the same time. It is important that patients are medically evaluated and cleared by their internist to assure their heart and lungs can tolerate a longer surgery and general anesthesia. Medical conditions such as heart disease, diabetes, or pulmonary conditions are likely contraindications for prolonged cosmetic elective procedures such as a facelift combined with rhinoplasty.

Retracted Ala after rhinoplasty

When evaluating my Houston revision rhinoplasty patients, many may have a pinched tip and retracted nostrils due to overzealous resection of cartilage with previous rhinoplasty. Retracted nostrils after rhinoplasty give a flared effect to the nostrils with too much nostril show on the lateral or profile view. Correction of retracted nostrils requires grafting with septal cartilage after septoplasty, ear cartilage, or rib cartilage. Alar strut or batten grafts in addition to alar rim grafts are necessary to bring the alar margin down. If retraction is severe, a composite graft from the ear may be required.
