Retracted Ala after rhinoplasty


When evaluating my Houston revision rhinoplasty patients, many may have a pinched tip and retracted nostrils due to overzealous resection of cartilage with previous rhinoplasty. Retracted nostrils after rhinoplasty give a flared effect to the nostrils with too much nostril show on the lateral or profile view. Correction of retracted nostrils requires grafting with septal cartilage after septoplasty, ear cartilage, or rib cartilage. Alar strut or batten grafts in addition to alar rim grafts are necessary to bring the alar margin down. If retraction is severe, a composite graft from the ear may be required.
