How much fat is repositioned with lower blepharoplasty?


This was a question recently asked by one of my Houston lower blepharoplasty patients.  With lower eyelid surgery, enough fat needs to be removed to improve the bags under the eyes.  However, if too much fat is removed, this can lead to a hollowed out effect.  The fat should be removed down to the level of the bony orbital rim.  Repositioning of the fat occurs by releasing the arcus marginalis which is at the bony rim.  Fat can be moved and repositioned over the rim or placed as a free graft over the rim.  This prevents the hollowed out apparance and allows for a better transitioin beteween the eyelid and cheek.
