Scars after lower eyelid surgery

Many of my Houston patients who come in for lower eyelid bags and are considering lower eyelid surgery are concerned about possible scars from the surgery. Lower eyelid surgery can be performed through a transconjunctival approach or through the skin. With the transconjunctival approach an incision is made on the inside of the eye to access the fat that causes the lower eyelid bags. This fat can then gently be teased out and possibly repositioned to create a better transition between the eyelid and the cheek. This would allow for scarless lower eyelid surgery. However, a small skin pinch may be necessary for lax skin of the lower eyelid after the fat is removed.
